oh, maybe something have change, I have had the same problem with rbd plugin.

can you try with:

-f raw -O raw

 ?

(code is in /usr/share/perl5/PVE/QemuServer.pm , sub qemu_img_format)

Looks like the qemu-img provided with pve-3.0rc2 is buggy: 
/usr/bin/qemu-img convert -t writeback -p -C -f iscsi -O iscsi 
iscsi://192.168.2.80/iqn.2010-09.org.openindiana:omnios:share/0 
iscsi://192.168.2.80/iqn.2010-09.org.openindiana:omnios:share/1 
Segmentation fault
